You can download the form on our website and fill it out on your computer or by hand.

The form must be completed in Slovak language.

Čestné vyhlásenie vlastníka nehnuteľnosti – declaration from the property owner.

The sample can be filled out manually by downloading it from our website.

The document must be completed in Slovak or Czech and certified by a Slovak or Czech notary, or personally signed at the foreign police department once submitting documents.

Nájomná zmluva – rental agreement.

It’s necessary to prepare a separate copy or a notarized copy of the agreement for submission to the police.

The document must be completed in Slovak or Czech and certified by a Slovak or Czech notary.

Potvrdenie ubytovacieho zariadenia – confirmation of accommodation.

Handed out when living in dormitories, hostels, hotels, etc.

The document must be completed in Slovak or Czech and certified with the seal of the institution.

Elektronický súhlas o zabezpečení ubytovania – electronic consent of the property owner to provide accommodation.

It is submitted directly by the property owner(s) via the Slovak Ministry of the Interior’s e-services portal.

It is also important to note:

1) Any proof of accommodation must be issued for a period of at least 6 months.

2) Proof of accommodation is not required if working in international transport.

3) In case a foreigner owns and occupies their own property, submission of proof of accommodation is not required.

Potvrdenie zo Sociálnej poisťovne – confirmation from the Social Insurance Agency.

Confirmation of the applicant’s inclusion in the register of insured persons as an employee in an employment relationship. This confirmation is issued by the branches of the Social Insurance Agency upon application (Žiadosť o vydanie potvrdenia), the form for which can be found on our website.

Please note: this confirmation must be no older than 30 days at the time of its submission to the Foreign Police.

Cestovný pas – international passport.

The passport must be valid at the time of documents submission.

Police officer will make a copy of the passport and return it to the applicant.

The original certificate must be apostilled in advance. You must have the original certificate with you or its notarized copy with a court translation into Slovak or Czech.

Alternatively, you can order a criminal record certificate electronically through the Diia app. You need to print it out and visit the Ukrainian consulate to have it certified. The consular document will be issued in Slovak and attached to the Ukrainian original. Such certificate does not require translation or an apostille.
