General rules and list of documents required for personally submitting an application to foreign police (cudzinecká polícia).
Applying for a residence permit via employment for holders of temporary protection (dočasné útočisko).
The main condition for applying for a residence permit under the simplified procedure is the continuous fulfillment of the purpose of the residence permit for at least 6 months. Therefore, employment must be continuous for 6 months immediately preceding the application for a residence permit.
1) Applicant’s form
Žiadosť o vydanie dokladu – application for the issuance of a residence permit card.
You can download the form on our website and fill it out on your computer or by hand.
The form must be completed in Slovak language.
2) Proof of accommodation in Slovakia
Čestné vyhlásenie vlastníka nehnuteľnosti – declaration from the property owner.
The sample can be filled out manually by downloading it from our website.
The document must be completed in Slovak or Czech and certified by a Slovak or Czech notary, or personally signed at the foreign police department once submitting documents.

Nájomná zmluva – rental agreement.
It’s necessary to prepare a separate copy or a notarized copy of the agreement for submission to the police.
The document must be completed in Slovak or Czech and certified by a Slovak or Czech notary.

Potvrdenie ubytovacieho zariadenia – confirmation of accommodation.
Handed out when living in dormitories, hostels, hotels, etc.
The document must be completed in Slovak or Czech and certified with the seal of the institution.

Elektronický súhlas o zabezpečení ubytovania – electronic consent of the property owner to provide accommodation.
It is submitted directly by the property owner(s) via the Slovak Ministry of the Interior’s e-services portal.
It is also important to note:
1) Any proof of accommodation must be issued for a period of at least 6 months.
2) Proof of accommodation is not required if working in international transport.
3) In case a foreigner owns and occupies their own property, submission of proof of accommodation is not required.
3) Confirmation of the employment period
Potvrdenie zo Sociálnej poisťovne – confirmation from the Social Insurance Agency.
Confirmation of the applicant’s inclusion in the register of insured persons as an employee in an employment relationship. This confirmation is issued by the branches of the Social Insurance Agency upon application (Žiadosť o vydanie potvrdenia), the form for which can be found on our website.
Please note: this confirmation must be no older than 30 days at the time of its submission to the Foreign Police.
4) Original passport
Cestovný pas – international passport.
The passport must be valid at the time of documents submission.
Police officer will make a copy of the passport and return it to the applicant.
5) Proof of no criminal record
Výpis z registra trestov Ukrajiny – criminal record certificate from Ukraine.
The original certificate must be apostilled in advance. You must have the original certificate with you or its notarized copy with a court translation into Slovak or Czech.
Alternatively, you can order a criminal record certificate electronically through the Diia app. You need to print it out and visit the Ukrainian consulate to have it certified. The consular document will be issued in Slovak and attached to the Ukrainian original. Such certificate does not require translation or an apostille.
This category of applicants is exempt from paying the administrative fee for document processing. The administrative fee is charged only for the issuance of the ID card (karta pobytu). The administrative fee amounts can be found on our website.
