General rules and list of documents required for submitting an application to foreign police (cudzinecká polícia).
Applying for a permanent residence permit based on marriage to Slovakia citizen (Trvalý pobyt na päť rokov) for citizens of non-EU countries.
Personal presence of the applicant is required. A translator may be presented.
1) Applicant’s form
Žiadosť o udelenie trvalého pobytu na 5 rokov – application for a permanent residence for 5 years.
The form is generated automatically as you fill it out on the police website.

The form must be completed in Slovak language.
2) Confirmation of residence in Slovakia
Čestné vyhlásenie vlastníka nehnuteľnosti – declaration from the property owner.

The document must be completed in Slovak or Czech and certified by a Slovak or Czech notary, or personally signed at the migration police department once submitting documents.

Nájomná zmluva – rental agreement.
It’s necessary to prepare a separate copy or a notarized copy of the agreement for submission to the police.

Potvrdenie ubytovacieho zariadenia – place of residence confirmation.
Handed out when living in dormitories, hostels, hotels, etc.
The document must be completed in Slovak or Czech and certified with the seal of the institution.

Čestné vyhlásenie o vlastníctve nehnuteľnosti – declaration of real estate ownership.
Submitted if a foreigner owns his own real estate.

Also important to note:
Residence addresses of a spouse-citizen of Slovakia and foreigner applying, must match! If the permanent place of registration (trvalý pobyt) of a citizen of Slovakia doesn’t match with the actual address of the planned place of residence he/she will need to register by the place (obtain prechodný pobyt) of joint residence.
3) Foreigner living finances
Výpis z osobného účtu za posledné tri mesiace – certificate of account movements for the last 3 months.
In case registering account in a foreign bank it’s recommended to prepare the document in English since a court translation into Slovak will be needed. The exception are certificates issued in Czech.
In this case, the account balance must be at least 12 subsistence minimums (as of 15.07.2024 approximately 3300 euros). Account balance must be higher than the specified amount throughout the entire period of consideration of the case by the police.

Potvrdenie zamestnávateľa o výške vyplácanej mzdy – confirmation from the employer about the employee’s salary.
When applying for permanent residence permit (trvalý pobyt) by a foreigner over 18 years of age, in addition to the above conservative options, you can also submit similar documents on behalf of your spouse. In this case, it’s additionally necessary to provide an obligation of financial and material support from the guarantor (Čestné vyhlásenie o poskytnutí finančného a hmotného zabezpečenia).
You can also provide your spouse’s employment contract (pracovná zmluva). When submitting, a notarized copy of the agreement is provided. Here it’s also necessary to provide an obligation of financial and material support from the guarantor (Čestné vyhlásenie o poskytnutí finančného a hmotného zabezpečenia).
When applying for permanent residence permit (trvalý pobyt) by a foreigner below 18 years of age, the financial security will be one of the above listed documents on behalf of the guarantor. Here it’s also necessary to provide an obligation of financial and material support from the guarantor (Čestné vyhlásenie o poskytnutí finančného a hmotného zabezpečenia).
4) One color photo
Photos can be made in almost every shopping center or photo studio.
It’s recommended to crop photos measuring 3 x 3,5 cm and attaching to the applicant’s form right away, so when filing a police report, you don’t waste time on that.
5) Original passport
Cestovný pas – international passport.
The passport must be valid at the time of documents submission.
Police officer will make a copy of the passport and return it to the applicant.
6) Confirmation of the grounds for obtaining permanent residence permit for 5 years
Sobášny list – marriage registration certificate.
If the marriage took place in Slovakia – a marriage certificate (sobášny list) isn’t provided.
If the marriage wasn’t concluded in Slovakia, it’s necessary to affix an apostille to the original document. Make a copy of the document from a Slovak notary. Next, this copy must be translated by a a court translator into Slovak. The resulting translation will be certified by the translator’s seal.
If the registry doesn’t have a translator from the original language of the document, then you can use the services of a court translator in the Czech Republic. In this case, documents translated into Czech will be accepted by the aliens police.
A birth certificate will be a confirmation for a child to obtain permanent residence permit for 5 years.
Rodný list – birth certificate.
it’s necessary to affix an apostille to the original document. Make a copy of the document from a Slovak notary. Next, this copy must be translated by a a court translator into Slovak. The resulting translation will be certified by the translator’s seal.
If the registry doesn’t have a translator from the original language of the document, then you can use the services of a court translator in the Czech Republic. In this case, documents translated into Czech will be accepted by the aliens police.
7) Statement about child’s unmarried status
Čestné vyhlásenie, že dieťa je slobodné – declaration of non-marriage.

Important: document is filled out for a child of any age!
8) Consent of the second parent
Súhlas rodiča – confirmation of the second parent’s consent.
Confirmation is the written consent of the second parent agreeing with his child obtaining permanent residence permit. Such a document can be certified either by a Slovak notary, in which case no additional actions are required, or certified by a foreign notary, in which case it’s certified by an apostille and requires translation into Slovak or Czech by a court translator..
9) Permanent registration address confirmation (trvalý pobyt) of Slovakia citizen
Občiansky preukaz – identification card.
Document must be valid at the time of submission.
You must submit a notarized copy to the police or present the original document when applying.
or
Potvrdenie obce o trvalom pobyte – certificate of permanent residence from the city/village/district administration.
10) Confirmation of no crime record
Výpis z registra trestov z krajiny občianstva – no crime record from the country of citizenship.
You must have with you the original certificate or its notarized copy with a court translation into Slovak or Czech. The original certificate must also be apostilled.
Important!
If the applicant has lived in any country for more than 90 days in 6 months period over the last 3 years or has not lived, but has the appropriate mark or national visa D in his current passport, he will additionally need to present no crime record from all those countries that he visited.
Such a certificate must first be apostilled and translated into Slovak or Czech.
If the applicant holds two or more citizenships, there may appear a request to provide all valid passports.
At discretion of the police officer, there may also be a request to provide no crime records from all countries, of which the applicant is a citizen.
