General rules and list of documents required for restoring temporary residence permit/permanent residence card (karta pobytu) in case of its loss.
Submitted to the foreign police department at the current of residence. Presence of the applicant is required. A translator may be invited
1) Applicant’s form
Žiadosť o vydanie dokladu – application for obtaining the card.
The form is generated automatically as you fill it out on the police website.
It can also be filled out manually. For example, if any mistakes appear. Or download žiadosť o obnovenie prechodného pobytu on our website.
The form must be completed in Slovak language.
2) One color photo
Photos can be made in almost every shopping center or photo studio.
It’s recommended to crop photos measuring 3 x 3,5 cm and attaching to the applicant’s form right away, so when filing a police report, you don’t waste time on that.
3) Original passport
Cestovný pas – international passport.
The passport must be valid at the time of documents submission.
Police officer will make a copy of the passport and return it to the applicant.

It’s recommended to immediately make a report about the loss of your ID card to your local police department. Better to attach confirmation of application acceptance (a copy) by police to application for the new card issuance.
According to law, №404/2011 a penalty can be given for ID card loss. However, the police officer receiving the documents will decide on the fine individually.
