General rules and list of documents required for restoring temporary residence permit/permanent residence card (karta pobytu) in case of its expiration.
Submitted to the foreign police department at the current address of residence. Presence of the applicant is required. A translator may be invited.
If the ID card was issued for a shorter period than the residence permit (povolenie na pobyt), then it must be replaced. Usually such a situation happens if previously, the applicant submitted an application for obtaining/renewal of a residence permit and provided confirmation of residence in Slovakia for a shorter period than the period of the received residence permit.
1) Applicant’s form
Žiadosť o vydanie dokladu – application for obtaining the card.

The form must be completed in Slovak language.
2) Confirmation of residence in Slovakia
Čestné vyhlásenie vlastníka nehnuteľnosti – declaration from the property owner.

The document must be completed in Slovak or Czech and certified by a Slovak or Czech notary, or personally signed at the foreign police department once submitting documents.

Nájomná zmluva – rental agreement.
It’s necessary to prepare a separate copy or a notarized copy of the agreement for submission to the police.

Potvrdenie ubytovacieho zariadenia – place of residence confirmation.
Handed out when living in dormitories, hostels, hotels, etc.

Čestné vyhlásenie o vlastníctve nehnuteľnosti – declaration of real estate ownership.
Submitted if a foreigner owns his own real estate.

Also important to note that proof of residence must be issued for a period of at least 6 months.
3) One color photo
Photos can be made in almost every shopping center or photo studio.
It’s recommended to crop photos measuring 3 x 3,5 cm and attaching to the applicant’s form right away, so when filing a police report, you don’t waste time on that.
4) Original passport
Cestovný pas – international passport.
The passport must be valid at the time of documents submission.
Police officer will make a copy of the passport and return it to the applicant.

Important: according to the requirements of foreigners law №404/2011 application for issuance of a new ID card must be submitted earlier than 30 days before its expiration date.
