It’s necessary to prepare a separate copy or a notarized copy of the agreement for submission to the police.

Potvrdenie ubytovacieho zariadenia – confirmation of accommodation.

Handed out when living in dormitories, hostels, hotels, etc.

Elektronický súhlas o zabezpečení ubytovania – electronic consent of the property owner to provide accommodation.

It is submitted directly by the property owner(s) via the Slovak Ministry of the Interior’s e-services portal.

It is also important to note:

1) Any proof of accommodation must be issued for a period of at least 6 months.

2) The guarantor’s and child’s addresses must match.

In case of having insurance with a Slovak company, certificate (Potvrdenie o poistnom vzťahu) must be obtained from your insurance company office in paper form with the stamp and signature of the employee.

If child is insured by a foreign insurance company, the certificate must be received in paper form with the seal and signature of a company employee. Next, this copy must be translated by a court translator into Slovak. The resulting translation will be certified by the translator’s seal. If the registry doesn’t have a translator from the original language of the document, then you can use the services of a court translator in the Czech Republic. In this case, documents translated into Czech will be accepted by the  Foreign Police.

Cestovný pas – international passport.

The passport must be valid at the time of documents submission.

Police officer will make a copy of the passport and return it to the applicant.

Čestné vyhlásenie, že dôvody udelenia prechodného pobytu naďalej trvajú – declaration from the guarantor that the grounds granting temporary residence are still applying.
