Handed out when living in dormitories, hostels, hotels, etc.

The document must be completed in Slovak or Czech and certified with the seal of the institution.

Elektronický súhlas o zabezpečení ubytovania – electronic consent of the property owner to provide accommodation.

It is submitted directly by the property owner(s) via the Slovak Ministry of the Interior’s e-services portal.

It is also important to note:

1) Any proof of accommodation must be issued for a period of at least 6 months.

2) The addresses of the guarantor and the applicant for reunion must match.

Cestovný pas – international passport.

The passport must be valid at the time of documents submission.

Police officer will make a copy of the passport and return it to the applicant.

Rodný list – birth certificate.

It’s necessary to affix an apostille to the original document. Make a copy of the document by a Slovak notary. Next, this copy must be translated by a court translator into Slovak. The resulting translation will be certified by the translator’s seal.

If the registry doesn’t have a translator from the original language of the document, then you can use the services of a court translator in the Czech Republic. In this case, documents translated into Czech will be accepted by the  Foreign Police.

The document must be completed in Slovak or Czech and certified by a Slovak or Czech notary, or personally signed at the foreign police department when submitting documents.

Important: the document is filled out for a child of any age!

Výpis z registra trestov z krajiny občianstva – criminal record certificate from the country of citizenship.

The certificate is required starting from the age of 14.

You must have the original certificate with you or its notarized copy with a court translation into Slovak or Czech. The original certificate must be apostilled in advance.

Important!

If the applicant has lived in any country for more than 90 days in 6 months period over the last 3 years or has not lived, but has the appropriate mark or national visa D in his current passport, he will additionally need to present no crime record from all those countries that he visited.

Such a certificate must first be apostilled and translated into Slovak or Czech.

If the applicant holds two or more citizenships, there may appear a request to provide all valid passports.

At discretion of the police officer, there may also be a request to provide no crime records from all countries, of which the applicant is a citizen.

Súhlas rodiča – confirmation of the second parent’s consent.

Confirmation is the written consent of the second parent with his/her child getting a residence permit in Slovakia. Such a statement can be certified either by a Slovak notary, in which case no additional actions are required, or certified by a foreign notary, in which case it’s certified by an apostille and requires translation into Slovak or Czech by a court translator.

Confirmation isn’t required if we are talking about the reunion of a common child of the guarantor and the spouse of the guarantor, i.e. parents are legally married. Accordingly, it will be necessary to provide proof of marriage – Sobášny list (marriage registration certificate).
